What is the problem?

(I know its about treating Parkinson's but what is the context here - why does MAX foundation need funds and for whom) There are a number of patients that come in for the treatment of Parkinson’s/Dementia from the lower economic strata.  The Max Foundation deals with a range of diseases/initiatives and does not have any allocation per se for these specific areas of Parkinson’s/Dementia treatment.  The funds would be specifically for patients who are not able to afford these treatments.

What is the treatment for exactly, what does it involve, how long will it take, who are the patients, what is their economic background and criterion for selection (TO GET THESE FUNDS they may say we do it only for those in this income bracket, in this age group, and these geographies)

F-DOPA PET Scan

This treatment is approved by the FDA and is used in the diagnosis of Parkinson’s Disease and other Parkinsonian syndromes.  This procedure uses a radioactive tracer that can help to visualize the nerve endings of dopaminergic neurons.  In PD patients that uptake of the tracer in the brain is reduced.  This imaging technique can be used to distinguish the parkinsonian condition, characterized by degeneration of the dopaminergic neurons, from other conditions which are not characterized by the degeneration of dopaminergic neurons, such as essential tremor.  When clinical examination is not sufficient, this scan is very useful to differentiate between the different clinical possibilities that the doctor can pursue.

FDG PET/CT Scan

In patients with different sub-types of Dementia, the FDG PET/CT shows distinct spatial patterns of metabolism in the brain and can help clinicians to achieve better diagnostic accuracy, earlier in the course of the disease, and can provide physicians with more-precise information regarding the prognosis and management of dementias. A negative amyloid brain PET is useful to exclude Alzheimer’s Disease as a possible cause of dementia.

Deep Brain Stimulation (DBS)

DBS is the most commonly performed surgical treatment for Parkinson’s Disease (PD).  It is a neurosurgical procedure that uses implanted electrodes and electrical stimulation to treat movement disorders associated with Parkinson’s Disease (PD), essential tremor, dystonia, and other neurological conditions.  DBS involves making small holes in the skull to implant the electrodes into brain tissue. The procedure also includes surgery to implant a pacemaker-like device under the skin in the chest.  This device contains the batteries needed for DBS.  DBS has been used in people whose symptoms are not controlled with medicines.  DBS regulates abnormal electrical signaling patterns in the brain – in PD these signals become irregular and uncoordinated which leads to motor symptoms.  This treatment is regularly used also for the Young-onset PD.  DBS has been approved by the FDA.

As mentioned before, all the patients will be from the lowest economic strata, and the determination of this will be done by Dr. J. D. Mukherjee, Vice Chairman & Head - Neurology at the Max Super Speciality Hospital in Saket, New Delhi.  Dr. Mukherjee will be responsible for ensuring that the funds are allocated to needy and deserving patients only.
